Send the following information:
Copies of
1) Passport & personal information page
2) Address page in Passport (if you have one)
3) USA Visa Page
4) Copy of I-94 (white card) and all J-1 forms(if applicable).
5) Previous 3 Months of Bank Statements or Form I-134
* Bank account information must be listed in the applicants name and in
English with USD currency.
* If sponsored by a US Citizen, a form I-134 must be completed and notarized.
6) Reasons for Visa Extension
* On a separate paper, handwrite the exact reasons you are applying for an
extension/change of status. Please click for exact requirements. Without these items

8) Immigration now requires a copy of an airline ticket home. Ask us how we can help
without you having to purchase a ticket and pay penalties to change or cancel.
9) New Fees as of 08/10/2007:
  Regular Application: $445.00 -- Must be received more than 30 days prior to expiration
  Urgent Application:    $495.00 -- Must be received 29 to 15 days prior to expiration
  EXpress Application:$545.00 -- Within 14 days of expiration
  Each additional family member applicant $50.00
Remember:
As a tourist, you have no legal approval from INS to study, work or enlist in the US military.

Please be sure to include all information. Any omissions might result in a
"DENIAL"!!!   A "REQUEST FOR EVIDENCE" means you will need to resubmit
additional information and pay a re-application fee of $75.00. Also, please note that
we are only acting as an agent on your behalf with immigration. Their answers are
final and we have no bearing on the outcome of your application.
